I, the undersigned participant/parent or guardian of the participant, recognize and acknowledge that use of the Youth Xtreme Circuit Laboratory  LLC  involve inherent risks, including but not limited to the risk of serious physical/mental injury, including permanent disability or death, and severe social and economic losses which might result from participant action, inaction, negligence of others, rules of play, or the condition of the premises or any equipment used thereon. Further, I understand that there may be other risks not known or reasonably foreseeable at this time and that such risks shall be assumed by the undersigned. In consideration of such risks, known or unknown, I agree as follows:
I expressly agree and promise to accept and assume all the risks existing in this activity. My/my child’s participation in this activity is purely voluntary and I elect, in spite of all the risks, to participate and/or to allow my child to participate. I assume all the foregoing risks and accept personal responsibility for any damages or injuries that I or my child may suffer.
On behalf of myself, my children, my parents, my heirs, assigns, personal representatives I hereby voluntarily release, waive, forever discharge, and agree to indemnify and hold harmless Youth Xtreme Circuit Laboratory LLC and each of its respective employees, managers and coaches from any and all liability for any and all claims, demands of causes of action which are in any way connected with my or my child’s use of the Facility interior/exterior. This release of liability, indemnity and hold harmless applies equally to losses, damages, or injuries caused or alleged to be caused in whole or in part by the negligence of the Youth Xtreme Circuit Laboratory LLC, its respective employees, managers and coaches. I further agree to release, waive and discharge, and covenant not to sue Youth Xtreme Circuit Laboratory LLC and each of its respective employees, managers and coaches for any claims, demands, or actions whatsoever arising out of any damage, loss, or injury incurred on or to me or my child as a result of my participation, or my child’s participation, in activities involving the Facility.
I further understand that I am legally responsible for actions of my child, including, but not limited to, any damage to private or public property.
I hereby certify that I have read and am familiar with the Facilities Rules and Regulations and will comply with conditions set forth therein.
I acknowledge and certify that I am at least 18 years old, or the parent or legal guardian of the participant under 18, and I agree my child will be supervised by a member 15 or older if the member is 10 years or younger at ALL times. I hereby provide that Youth Xtreme Circuit Laboratory LLC has my permission to administer basic First Aid and I authorize Youth Xtreme Circuit Laboratory LLC employees, managers and coaches to contact 911 or other emergency personnel as needed.
Furthermore, any cancellation of my memberships will take place on the members enrollment anniversary day.  If I'm found allowing non-members in the facility I will receive; one warning, second offense is a one time penalty fee of $50 charge to the my/members account, thirdly my/members memberhship will be terminated and may be reconsidered after six months of termination date.  Lastly, your giving Youth Xtreme Circuit Laboratory LLC permission to share social media content of your minor for promotional purposes.
By signing this document, I acknowledge that if anyone is hurt, or property is damaged during participation in this activity a court of law may find me to have waived my and my child’s right to maintain a lawsuit against Youth Xtreme Circuit Laboratory LLC and each of its respective employees, landlord, managers and coaches on the basis of any claim from which I have released them herein.
I HAVE HAD SUFFICIENT OPPORTUNITY TO READ THE ENTIRE DOCUMENT. I HAVE READ AND UNDERSTOOD IT, AND I AGREE TO BE ULTIMATELY BOUND BY ITS TERMS INDEFINITELY.
